Child participation waiver

One waiver covers each child listed below. Add every child who will play. Ages 18 months to 9 years. Kids under 3 must be accompanied by a parent or guardian in the play area at all times.

Waiver and release

I am the parent or legal guardian of each child listed above, and I give permission for them to take part in Little Kingdom Play activities, including the Little Builders and Little Royals play areas, toys, crafts and games.

I understand that active play carries some risk of bumps, scrapes, falls and other injuries. I understand that Little Kingdom Play hosts run the activities but do not replace a parent's supervision. A parent or guardian will stay on site for the whole event, and any child under 3 will have a parent or guardian with them in the play area at all times.

To the extent allowed by Idaho law, I release Little Kingdom Play LLC, its owners and staff from claims for ordinary injuries that may happen during normal play, except those caused by gross negligence or willful misconduct.

If my child needs urgent medical care and I cannot be reached right away, I authorize Little Kingdom Play staff to call 911 and seek emergency treatment.

I agree my children will follow the host's play rules, and that a child may be asked to take a break if play becomes unsafe.

I have read and agree to the waiver and release above for every child listed. I understand that kids under 3 must be accompanied by a parent or guardian at all times. Optional: Little Kingdom Play may share party photos that include my child.
